How to Become A Patient?
Before visiting the Clinic, there are a few steps that need to be completed first to make sure you are eligible and prepared for your appointment.

Once you complete steps 1 and 2, you are ready to make an appointment. Be sure to bring your required documents!

The purpose of a screening appointment is to gather all of the important information from our patients so we can begin to provide them with care. This covers the information your providers need, consent forms, and the information the clinic evaluates and verifies to ensure you are eligible.

Eligibility screening are by appointment only at the Main Clinic Facility in Kilmarnock during the following hours:
Monday: 9am - 12am
Tuesday: 2pm - 4pm
Wednesday: 9am - 12am
Thursday: 2pm - 4pm
Friday: 9am - 12am

If you are a new patient, screening appointments can take about an hour.
If you are a returning patient, screening appointments can take as little as 15 minutes. It depends on how much information needs to be updated in our records and how many forms need to be re-done.

To every screening appointment, patients need to bring their:
I.D. (driver’s license, passport, etc.)
Insurance cards
Financial information - one of three things
3 most recent months of paystubs
Tax 1040 form
Social Security Letter
If you do not have any income, your screener will prepare a Letter of Support and a Declaration of Income/No Income. If you are self-employed, a Declaration of Income will be prepared. After these forms are filled out, they will be notarized.
If you have a spouse (a partner you are legally married to), you need to bring their financial information as well. Even if they are not a patient or don’t plan to become a patient, your Federal Poverty Level (FPL) is based on your income and household size. This means all income supporting the household needs to be documented.

If your last screening was done over a year ago, then yes. Everyone needs to renew their screening every 12 months.
Every patient has a screening expiration date that is exactly a year after their previous screening appointment. This information renewal needs to be done before you can see a provider again. This is necessary to update any financial information, insurance information, and/or personal information.
