We provide sports physicals to children who attend:
Lancaster County Schools
Northumberland County Schools
Middlesex County Schools
Westmoreland County Schools
Richmond County Schools

Both of these packets will be available at the schools of the 5 counties and the NNMFHC packets will be available at the high schools of the 5 counties, the Lancaster Public Library, and the NNMFHC Main Office.
Both of these packets need to be filled out before your appointment. If you do not have the forms or have any questions, please arrive 30 minutes early.

Requirements for Appointment
Guardian over 18 years of age (If not child’s parent or legal guardian, then must have verbal or written consent for treatment from parent)
Child’s Insurance Card(s)
Parent/Guardian’s ID Card
NNMFHC and VHSL completed packets
