However, if you’re still confused, here are the specific steps:

    1.  Call the clinic and let them know what services you are looking for.

    2. Make a screening appointment.

**If you are unclear on the documentation you need to bring, the staff member on the phone can let you know.

    3. Attend the screening appointment and fill out patient information forms, financial forms, insurance forms, sign permission and acknowledgment forms, and take a short survey.

**Please note some forms can be completed ahead of time. Click here to view.

  • This information is used to provide general patient information for your healthcare providers, general demographic information for our records, and financial information for your eligibility. A survey also is used to check in with your well-being outside of the clinic.

Collectively, this information may seem like a lot, but all of it is necessary to help NNMFHC give you the highest quality of care.

  • The purpose of a screening appointment is to gather all of the important information from our patients so we can begin to provide them with care. This covers the information your providers need, consent forms, and the information the clinic evaluates and verifies to ensure you are eligible.

  • Eligibility screening are by appointment only at the Main Clinic Facility in Kilmarnock during the following hours:

    • Monday: 9am - 12am

  • If you are a new patient, screening appointments can take about 60 minutes.

If you are a returning patient, screening appointments can take as little as 15 minutes. It depends on how much information needs to be updated in our records and how many forms need to be re-done.

  • To every screening appointment, patients need to bring their:

    • I.D. (driver’s license, passport, etc.)

    • Insurance cards

If you do not have any income, your screener will prepare a Letter of Support and a Declaration of Income/No Income. If you are self-employed, a Declaration of Income will be prepared. After these forms are filled out, they will be notarized.

If you have a spouse (a partner you are legally married to), you need to bring their financial information as well.
