We provide sports physicals for children that attend:

Northumberland County Schools
Middlesex County Schools
Westmoreland County Schools
Richmond County Schools

Both of these packets will be available at the high schools of each county and NNMFHC Patient Information Packets are available at the NNMFHC Main Office in Kilmarnock.
Both of these packets need to be filled out before your appointment. If you do not have either forms, arrive 30 minutes early and we will provide you with the packets for you to fill out. Students must also bring any insurance cards and must have a guardian present.
